A Case of Achalasia Cardia operated Laparoscopically

A condition in which the muscles of the lower part of the oesophagus fail to relax, preventing food from passing into the stomach
Case Scenario
A 50 year old woman came with complaints of difficulty in swallowing liquids since the past many years. She underwent investigations which revealed that she had a mega-esophagus due to a rare condition called Achalasia Cardia.
What is Achalasia Cardia?
Achalasia Cardia is a rare condition which affects the esophagus (food pipe). Achalasia cardia is a primary esophageal motility disorder with involvement of the Auerbach’s intermuscular plexus. Achalasia cardia is considered a very rare disease, its incidence rate is 10 cases per 100000 population, and morbidity rate is 1 per 100000 population. Achalasia cardia is diagnosed in adults most frequently in the age group of 25 to 60 years. Although the disease was first described by Thomas Williams, an English doctor, in 1674, the etiology of achalasia cardia still remains unknown. Genetic theory is one of the possible theories considered as the pathogenesis of achalasia cardia.
Patient Management through Laparoscopic Surgery –
The patient could not drink liquids and had dysphagia due to Achalasia Cardia. The patient was advised surgery for the same. Traditionally this surgery is done with a large incision on the abdomen or through chest but due to newer technologies like laparoscopy it can be done with small (0.5 cm – 1 cm) incisions on the abdomen.
The patient underwent successful laparoscopic Heller’s myotomy at VishwaRaj Hospital by Dr Sushil Deshmukh, Laparoscopic

What is Heller’s Myotomy?
Heller myotomy is a surgical procedure in which the muscles of the cardia (lower esophageal sphincter or LES) are cut, allowing food and liquids to pass to the stomach.
After the surgery patient started walking within 6 hrs. She is now able to drink liquids after one day of surgery. The condition of the patient has improved after the surgery. Laparoscopic Surgery has many advantages especially in such patients which require big incision on the abdomen or chest and is very painful surgery.
Advantages of Laparoscopic Surgery -
- 1. Very small scars
- 2. Stay for fewer days in the hospital (in traditional methods, the recovery time of a patient was 4 to 8 weeks and hospital stay was of 1 or more than a week, but in laparoscopic surgery, recovery time is 2 to 3 weeks and hospital stay is of only 2 nights).
- 3. Less tissue cutting.
- 4. Less pain while scar heals – lesser use of pain medications.
- 5. Get back to normal life sooner.
- 6. Quick healing as the scar is small.
- 7. As exposure of internal organs gets reduced for external contaminants, hence the risk of infections reduced.
