Case Scenario
A case of 31 years female G3A2L0 with 34 weeks of gestation on 01/12/2020 with bad obstetric history and cervical incompetence.
She is a registered case of VRH (OBSTETRIC AND GYNAECOLOGY DEPARTMENT)
She presented to VRH casualty with complains of leaking per vaginum and threatened pre term labour
Obstetric history- G3A2L0
A1- was 2 and a half month spontaneous abortion D & E done. A2- was 5 month PPROM abortion induced and delivered.
G3- present pregnancy of 34 weeks of gestation
Great Efforts
Management during delivery –
- Prophylactic Os tightening was done at 16 weeks in view of bad obstetric history and cervical incompetence.
- Patient again presented at 22 weeks with funneling of cervix and bulging of fetal membranes upto the internal Os.
- This emergency was managed with Emergency Os tightening and was later kept on complete bedrest and supportive management with progesterone and tocolytics.
- Strict watch kept on her Antenatal care.
- She again presented with leaking per vaginum and thretend pre term labour at 34 weeks of gestation.
- Pt monitoring done very closely.
- Fetal monitoring done continuously
- In view of pre term baby and bad obstetric history, decision of delivering the patient by cesarean section taken.
- Procedure was uneventful.
- Baby cried immediately after birth and shifted to NICU for further care.
